Description

Minirin® nasal spray is manufactured by Ferring Pharmaceuticals. This medication's aim is to regulate the water retention in the body and reduce urine production. Minirin® nasal spray should only be used in patients where orally administered formulations are not feasible. The active substance comprised in Minirin® nasal spray is Desmopressin (DDAVP), a synthetic structural analog of vasopressin (antidiuretic hormone or ADH). Used during an antidiuretic hormone replacement therapy.

Indications:

- management of cranial diabetes insipidus, followed by abnormal production of urine (polyuria) and constant thirst (polydipsia). Do not confuse with diabetes mellitus (sugar diabetes).

- primary nocturnal enuresis (bed-wetting) in patients who are able to concentrate urine normally and have not responded to safer treatments (alarms);

- nocturia associated with multiple sclerosis where other treatments have failed.

A single dose of Minirin® nasal spray can also be used as a diagnostic test to establish if the kidneys are able to concentrate urine.

Although Minirin® nasal spray aids in decreasing urine overproduction, it does not cure the aforementioned diseases.

MOA (mechanism of action):

DDAVP is a potent antidiuretic which may lead to water intoxication and/or hyponatremia. It emulates the action of vasopressin, a natural antidiuretic hormone. Desmopressin is also a strong vasopressin receptor 2 (V2 receptor) agonist, therefore it has an effect on the mechanisms of urine concentration and maintenance of water homeostasis in the body. Namely, it increases water permeability in renal tubular cells, thereby inhibiting urine overproduction and increasing its concentration. In addition to its antidiuretic properties, Desmopressin also has vasoconstricting activity.

Dosage

Minirin® nasal spray should be sprayed into nostrils.

The nasal spray pump can only deliver doses of 0.1 mL (10 micrograms) or multiples of 0.1 mL. If doses other than these are required, the rhinal tube delivery system may be used.

Nocturnal enuresis (bed-wetting):

Start with diffusing 1 spray (10 micrograms) into each nostril daily at bedtime. Over time, your doctor will adjust your dose depending on your response and severity of condition. The daily dose should be divided in 2-3 doses. Use the spray for up to 3 months.

The average recommended dose for children aged 3 months to 12 years varies from 5 to 30 micrograms daily.

The dose for adults and children over 12 usually varies from 10 to 40 micrograms daily.

Avoid drinking fluids 1 hour before and 8 hours after using Minirin® nasal spray. You can drink no more than a few sips of water during this period.

The maximum dose is two sprays into each nostril (40 micrograms).

Cranial diabetes insipidus:

The dosage, as well as the course duration, must be determined by a doctor for each individual patient.

For adults the daily dose usually varies up four sprays (40 micrograms).

For children the daily dose usually varies up to two sprays (20 micrograms).

How to use Minirin® nasal spray:

1. Remove protective cap from the bottle.

2. The spray pump must be primed prior to the first use. To prime pump, press it down 4 times.

3. Once primed, the spray pump delivers 10 micrograms of medication each time it is pressed. To ensure dosing accuracy, tilt bottle so that dip tube inside the bottle draws from the deepest portion of the medication. To administer a 10-microgram dose, place the spray nozzle in nostril and press the spray pump once. If a higher dose has been prescribed, spray half the dose in each nostril. The spray pump cannot be used for doses less than 10 micrograms or doses other than multiples of 10 micrograms.

4. Replace the protective cap on the bottle after use. The pump will stay primed for up to one week. If the product has not been used for a period of one week, re-prime the pump by pressing once.

5. Any solution remaining after 25 doses should be discarded since the amount delivered thereafter may be substantially less than 10 mcg of drug. Do not transfer any remaining solution to another bottle.

Precautions:

Before using Minirin® nasal spray alcohol abuse should be excluded.

Minirin® nasal spray is not for injection.

Minirin® nasal spray is for intranasal use only. Do not get it into your eyes, mouth, ears and other parts of the body.

Minirin® nasal spray should only be used in patients where orally administered formulations are not feasible.

Minirin® nasal spray should be used with caution in patients with habitual or psychogenic polydipsia (resulting in urine production exceeding 40mg/kg/24hours) who may be more likely to drink excessive amounts of water, putting them at greater risk of hyponatremia.

Avoid excessive fluid intake during the Minirin® nasal spray treatment course as this may lead to a build-up of water in your body.

Missing a dose

If you miss a dose you should take it as soon as you remember about your missing. If it is time for the next dose you should continue your regular dosing schedule. Do not take a double dose.

Overdose

Do not exceed the doctor's recommended dose. The symptoms of Minirin® nasal spray overdose may include confusion, drowsiness, continuing headache, nausea or vomiting, rapid weight gain, and convulsions in severe cases. If any of these occurs and you are feeling unwell, you should seek emergency medical attention or contact your healthcare provider immediately.

Storage

Store at a room temperature from 20 to 25 degrees C (68 to 77 degrees F) away from moisture and heat. Throw away the tube after the expiration date. Keep away from children.

Side effects

The most common side effects associated with the use of Minirin® nasal spray are:

  • nasal congestion
  • runny or blocked nose
  • sneezing
  • facial pressure or pain
  • fever
  • trouble sleeping or nightmares
  • nervousness
  • headache
  • nose bleed
  • upper respiratory tract infection
  • stomach pain or nausea
  • allergic reactions (difficulty breathing, rash, itching, swelling on the face, lips, tongue)

Side effects occurrence does not only depend on the medication you are using, but also on your overall health and other factors.

Contraindications

Do not take Minirin® nasal spray if you are allergic to Desmopressin or Vasopressin.

Do not take Minirin® nasal spray if you are allergic to any of the other ingredients comprised in Minirin® nasal spray.

Do not combine Minirin® nasal spray with the use of diuretics.

Do not take Minirin® nasal spray if you currently have a nasal infection, blocked up or runny nose.

Do not take Minirin® nasal spray if you have anuria, moderate and severe renal insufficiency (creatinine clearance below 50ml/min), heart or blood vessel disease, hypertension, cystic fibrosis, syndrome of inappropriate antidiuretic hormone secretion (SIADH), polydipsia or any medical condition causing fluid and/or electrolyte imbalance or any disease of the blood clotting cells (platelets).

Do not use Minirin® nasal spray if you are taking tricyclic antidepressants (clomipramine, imipramine, desipramine), SSRIs (citalopram, paroxetine, sertraline), antipsychotics (chlorpromazine), epilepsy drugs (carbamazepine), antidiabetics (chlorpropamide), high blood pressure medications (enalapril, perindopril, irbesartan), and non-steroidal anti-inflammatory drugs (aspirin and ibuprofen).

Do not take Minirin® nasal spray if you have hyponatremia (low levels of sodium in your blood).

Do not use Minirin® nasal spray in patients under 3 months of age.

It is recommended not to use Minirin® nasal spray in pregnant women unless considered absolutely necessary by your doctor.

It is strictly prohibited to breastfeed during the Minirin® nasal spray treatment course.
